Hospice care provides palliative and supportive services that address the physical, emotional, psychological, social, and spiritual needs of individuals facing a life-limiting illness, while also supporting their families and caregivers.
At Gateway Hospice Group LLC, we are dedicated to providing compassionate, patient-centered care focused on comfort, dignity, and quality of life. Our team works closely with patients and families to manage symptoms, provide emotional and spiritual support, assist with personal care needs, and offer guidance throughout every stage of the hospice journey.
Core hospice services, including nursing, social work, physician support, and bereavement counseling, are available 24 hours a day, 7 days a week to ensure patients and families receive the care and support they need when they need it most.
Nursing Services
Hospice nursing care is specialized care focused on providing pain management, physical comfort, and emotional support for individuals nearing the end of life, with an emphasis on comfort, dignity, and quality of life.
Hospice Aide Services
Hospice aides provide compassionate, hands-on personal care to hospice patients, helping support comfort, dignity, and daily well-being. Services may include assistance with bathing, oral care, foot care, shampooing, shaving, skin care, and changing bed linens.
Social Services
Social workers provide compassionate support and guidance to hospice patients and their families through education, counseling, community resources, and emotional support related to the terminal illness. They also assist with bereavement support and help families navigate the emotional and practical challenges that may arise throughout the hospice journey.
Counseling Services
Hospice counseling services provide emotional, spiritual, and supportive guidance to patients and their families as they prepare for the end-of-life journey. Counselors help individuals cope with the physical and emotional challenges of terminal illness while also supporting family members and caregivers through grief, loss, and bereavement.
Volunteer Services
Hospice volunteers provide compassionate support through a variety of services that may include companionship and direct patient support, assistance with office and administrative tasks, help with household chores, and other supportive activities that enhance comfort and quality of life for patients and families.
Chaplain Services
Hospice chaplains provide compassionate spiritual and emotional support to patients and their families during end-of-life care, offering comfort, guidance, prayer, and companionship while respecting each individual’s personal beliefs, values, and traditions.
Bereavement Services
Hospice bereavement services provide compassionate support to individuals and families coping with the loss of a loved one. These services offer comfort, guidance, education, and helpful resources to assist families as they navigate the emotional process of grief and healing.
Short Term Respite Care
Respite care provides temporary relief for family caregivers, allowing them time to rest, recharge, and attend to personal needs while ensuring their loved one continues to receive compassionate, quality care. This support helps reduce stress and prevent caregiver burnout.
Durable Medical Equipment (DME)/Supplies
Medical equipment like a hospital bed, wheelchairs, or walkers/medical supplies e.g., oxygen, bandages.
Rehabilitation Services/Dietary Counseling
Rehabilitation services may include physical, occupational, and speech therapy to support comfort and daily functioning. Dietary counseling provides personalized nutrition guidance focused on hydration and symptom management. These services are provided based on the hospice team’s assessment, the patient’s goals of care, and overall condition.
Our Procedure
A team member from Gateway Hospice Group LLC will schedule a convenient time to meet with the patient and their family or caregivers to discuss hospice services, answer questions, and explain the benefits of hospice care. During this meeting, we focus on providing guidance, support, and education to help families better understand the care process and available services.
Our team will also coordinate with the patient’s Primary Care Physician to review hospice criteria and determine eligibility for services. Once eligibility is confirmed, we work closely with patients and families to develop a personalized plan of care designed to provide comfort, dignity, and compassionate support every step of the way.
